This form was created for a series of lectures for which the organiser wanted an evaluation from the attendees. There is a set of 'topics', each one containing a number of questions.
The topics displayed here are chosen by admin during the design phase, as is the order in which the topics are displayed, and the row on which each topic appears. The questions for each topic are also chosen at this time.Validation of the form is performed in Javascript before the form is sent to the server.
The set of lectures is pre-set for the coming year; each lecture is added to the database by admin, who also sets the 'window' for comments (start date/time and end date/time) (see below).
For security reasons the two free-text inputs have been disabled in this demonstration. Otherwise this page is fully-functional.For this demo there is one lecture a month, being at or near the beginning of the month. Depending on the date (and time) you are viewing this page, the top row of the evaluation form may show lecture details and there will be a submit button at the bottom of the form. Alternatively, the top row may show a 'too late' message, along with the date of the next lecture; in this case the submit button will be missing.
Note that the start and end 'dates' also include a time element; this allows admin to set a 'window' for the comments to be made. Outside this window the evaluation form will not be available and a message to that effect is displayed.
To edit an entry, just click in the appropriate box. An action box appears. Make the changes and click on 'save'; the changes are added to the database and the page refreshed to show the new version.At the bottom of the page is an area for adding comments to the final report. Copy and paste selected comments into the boxes - or type in your own. You can put all the comments into one box or, alternatively, use a separate box for each comment.
Click the make PDF button.In this demo, the report is only displayed on the screen. (You could save the file from this page via the browser's File menu. It is also possible to have the file saved automatically to the server and/or emailed to selected people.
Use the browser's back button to return to the results page.
